“As a parent and advocate of an adult with disabilities, I have consistently worked on making sure my daughter had needed services entitled to her. As clients of NJ DDD, she and her husband had received services through the agency model where one agency was responsible for distributing goods and services to its clients. When NJ DDD switched over to the “fee for service” model, the transition was complicated and stressful to parents as well as the disabled and challenged clients. When advised by DDD that we would arbitrarily be given a coordination agency to manage my daughter’s needs, we were assigned House of Caring Hands, Inc. That assignment couldn’t have been more fortuitous. House of Caring Hands, Inc, under the direction of Fatu Jallah is the agency my daughter and her husband were assigned. The compassion and concern shown by Fatu and her excellent staff is second to none. They are always available to answer questions by phone and emails. They have the remarkable ability of efficiently cutting through red tape that is often associated with programs for the disabled. My wife and I could not be happier, knowing that our children are in the most capable hands to coordinate their much needed services. “House of Caring Hands Inc’s Support Coordination Team goes above and beyond! We are the Voice of the people we serve! Our Support Coordinators are professionals, and work well with all provider agencies and families to ensure the needs of the individuals.”

Barbara Smith (name of the individual has been changed to protect the individual), was assigned to  House of Caring Hands, Inc’s Support Coordination program in 2018. The assigned Support Coordinator worked tirelessly with the family of Barbara to link Barbara to needed services within her community to help meet her needs.

Barbara had lived with her parents all of Barbara’s life following the death of her parents. Barbara required 24 hours care, and transitioned to an emergency group home placement until another placement could be available.  Barbara living with her parents, had her own room, space and many more. Therefore, Barbara faced many challenges with being able to cope with her new environment. Barbara later had an opportunity to move out of the emergency placement into another group home with a new residential provider. Barbara was very excited for this opportunity with the intent of having a home-like environment that will provide for her a safety net. However, shortly after Barbara’s move into her residence with another provider, Barbara had an incident at her group home which then led to Barbara receiving nursing-home and rehab services for many months. Barbara’s version of what led to her requiring rehab has been an experience that she vows never to return to.  Barbara told her Support Coordinator that she did not want to return to the group home. Barbara wept every time she spoke about the incident and what she alleged to have caused her to require rehab. She stated, “ALL I WANT, IS TO LIVE ON MY OWN! Barbara’s sister expressed feelings of devastation, madness and still is today, and also vows to not have her sister returned. With that being said, Barbara’s Support Coordinator who works with House of Caring Hands, attended several meetings, advocating and being that voice for Barbara since the family was a bit emotional knowing the experience of Barbara (based on barbara’s narration of what occur). Barbara’s Support Coordinator explored all of her circle of network and community resources and with such hard work, Barbara transitioned into a Supervised Apartment with support services put in placed by the new provider. Barbara’s desires and Long-Term outcome has always been to have her own apartment and live on her own!

Since Barbara’s move into her own apartment and with the support of the new provider agency, Barbara has been flourishing and continued to be encouraged to utilize her resources both at home and within her community. Recently, with the support of her worker,  Barbara wrote an essay for the fire department. This essay has given Barbara the platform and the opportunity for Barbara to meet with the Mayor at her new residence where she will be honored for such a beautiful written essay with the support of her direct support staff and Support Coordinator. Thanks to the provider agency and support staff for embracing House of Caring Hands, Inc’s referral to help Barbara achieve her goal! Thanks to the Support Coordinator for working so hard and doing the things you do each day.
