COMMUNITY-BASED SUPPORT

House of Caring Hands, Inc is a DDD/Medicaid-approved provider. House of Caring Hands, Inc provides Community-Based Support in the following Counties, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Ocean, Passaic, Somerset and Warren.

Our Community Based Supports program provides direct support and assistance for participants, with or without the caregiver present, in or out of the participant's home, to achieve and or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community as outlined in the individual's service plan. 

Our Direct Support Professionals, (DSPs), serve as trained professionals providing support services to the individuals within our Community-Based Supports and Individual Supports programs. Our DSPs come with a worth of knowledge but are not limited to, community resources, basic medical needs, link/ escorting individuals served to community activities, problem-solving interventions, and more.
