House of Caring Hands, Inc offers a number of services to support our clients.

🖤 SUPPORT COORDINATION

Support Coordination is provided to individuals who are eligible for services through the NJ Division of Developmental Disabilities (DDD). Our Support Coordinators use individualized planning to assist individuals and their families. We strive to improve the quality of life for people with intellectual developmental disabilities through developed plans, linked resources and services. We adhere to the expectations of the Division of Developmental Disabilities, abiding to all of the state’s regulatory expectations, not limited to, the reporting of incidents to ensure the safety of the individuals served.

🖤 COMMUNITY-BASED SUPPORT

Our Community Based Supports program provides direct support and assistance for participants, with or without the caregiver present, in or out of the participant's home, to achieve and or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community as outlined in the individual's service plan.

🖤 INDIVIDUAL SUPPORT

Our Individual Supports program provides direct support and assistance for participants, with or without the caregiver present, in or out of the participant's home, to achieve and or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community as outlined in the individual's service plan.

🖤 TRANSPORTATION SUPPORT

House of Caring Hands, Inc provides transportation support and is a DDD/Medicaid-approved provider. House of Caring Hands, Inc provides Transportation Support in the following Counties, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Ocean, Passaic, Somerset and Warren.
