SUPPORT COORDINATION

House of Caring Hand, Inc is a release support coordination agency that approves its own Individualized Service Plan (ISP).

House of Caring Hands, Inc is a DDD/Medicaid-approved support coordination agency serving the following Counties, Atlantic, Bergen, Burlington, Camden, Mercer, Middlesex, Monmouth, Morris, Salem, Sussex, and Union.

As a person with a disability, you are able to choose the services you use and share how you want to be supported. Support Coordinators help navigate appropriate services that will best meet your needs.

Roles of Support Coordinators?

• The Support Coordinator manages Support Coordination services f each individual by performing 4 general functions: individual discover plan development, coordination of services, and monitoring.
• Scheduling and facilitating planning team meetings in collaboration with the individual; writing the PCPT and ISP; and distributing the ISP (and PCPT when the individual consents) to the individual, all team members, and the identified service providers; and reviewing the ISP through monitoring conducted at specified intervals.
• Monitoring and following up to ensure delivery of quality services, a ensuring that services are provided in a safe manner, in full consideration of the individual’s rights. 

• Reporting any suspected violations of contract, certification or monitoring/licensing requirements to the Division.
• Entering required information into the iRecord in an accurate and timely manner.
• Ensuring that individuals/families are offered informed choice of service provider. 

What Are The Qualifications For Becoming a Support Coordinator ?

•Bachelor's Degree or higher in any field.
• One year professional experience working with adult (21 or older) with intellectual/developmental disabilities, special education, mental health, counseling, guidance, social work or health and rehabilitative services.
• Complete trainings required by DDD before rendering services.
• Federal Criminal Background and Central Registry check at the point of hire.

What Is Support Coordination?

Support Coordination is a Division-funded service that assists individuals in gaining access to needed program and State plan services, as well as needed medical, social, educational and other services.

Eligibility for the Supports Program:

Individuals eligible for the Supports Program must meet the following criteria:
• At least 21 years old.
• Deemed eligible for Division services.

• Has and maintains Medicaid eligibility.
• Lives in an unlicensed setting – own home or family home.
• Is not currently enrolled in another HCBS or MLTSS program (including the CCW.

What Are The Minimum Contact Requirements? :

• Monthly contact documented on the Support Coordinator Monitoring Tool.
• Quarterly face-to-face contact documented on the Support Coordinator Monitoring Tool

• Annual home visit are required and must be documented on the Support Coordinator Monitoring Tool.
• Completed PCPT & ISP by 30 days from date the individual is enrolled into the Supports Program (and annually thereafter)
